What is Remote Patient Monitoring (RPM)?

Remote Patient Monitoring (RPM) is a technology to enable monitoring of patients outside of conventional clinical settings, which may increase access to care and decrease healthcare delivery costs. Key features of RPM, like remote monitoring and trend analysis of physiological parameters, enable early detection of deterioration; thereby, reducing number of emergency department visits, hospitalizations, and duration of hospital stays.

Incorporating RPM in chronic disease management can significantly improve an individual’s quality of life. It allows patients to maintain independence, prevent complications, and minimize personal costs. RPM facilitates these goals by delivering care right to the home. In addition, patients and their family members feel comfort knowing that they are being monitored and will be supported if a problem arises.

Why Medical Practice to consider RPM Program?

In addition to patient satisfaction and clinical benefits there is huge financial incentive. CMS (and some of states Medicaid, Private Payers) started reimbursing for RPM (approximately $200 per patient per month). Each Physician (or Qualified Healthcare professional) may earn an extra $400K with minimum supervision and training to clinical staff i.e. RN, MA (based on state regulation).

How does it work?

RPM program is like CCM program in fact same patient can be enrolled into RPM to manage vitals.

STEP 1

Enroll Patient

Identify and Enroll Patient into Program (Require Patient Consent) and qualify patient for free phone program.

STEP 2

Setup goals

Setup goals (i.e. BP value within certain range) & educate Patient how to use Device(s) at Home etc.

STEP 3

Communication

Review vitals and communicate with Patient as per program guidelines.

STEP 4

Disenroll/Re-enroll

Disenroll/Re-Enroll Patient as Required.
